This form is for retail operators, defined as a grocery store, deli or other business that sells packaged food products directly to consumers for home usage, to submit if they are eligible for a quarterly rebate based on their purchases of products from Sahlen Packing Co. during specified rebate periods. Once registered for the appropriate rebate, purchase your eligible Sahlen products. At the end of each quarter, provide your proof of purchase (distributor invoice) and proof of promotion where applicable via the electronic submission portal and be paid on the rebates you earned during the period.
